Instructions for Registration:
1. FIRST
- Print out and review the Schedule of Events to determine what
ticketed events you will be attending.
2. SECOND - If
paying by Credit Card (Visa/Mastercard), click on "Register
On-Line" and proceed with your registration. A
confirmation will automatically be sent to you. Please note that
you may register 1 ROTARIAN AND 1 SPOUSE/GUEST PER ON-LINE FORM.
IF YOUR SPOUSE/GUEST IS ALSO A ROTARIAN, PLEASE
HAVE HIM/HER REGISTER INDIVIDUALLY ON-LINE.
3. THIRD - If
paying by check, click on "Register and Pay By Check"
to print out the Off-Line Form. Complete the form, enclose your
check and mail it to the address shown on the form.
4. FOURTH - If
you are GOLFING in the PH Tournament, payment is collected with
your registration. A form for your entry (individual or
foursome) will be ready soon and posted on this page. All
participating golfers MUST BE REGISTERED for the Conference.
5. FIFTH - THE
REGISTRATION FEE OF $35.00 APPLIES TO EVERYONE ATTENDING THE
CONFERENCE - THIS INCLUDES SPOUSES AND GUESTS.
ABSOLUTELY NO EVENT TICKETS WILL BE ISSUED TO ANYONE NOT REGISTERED
FOR THE CONFERENCE!
6. SIXTH - Be
sure you have made your HOTEL RESERVATIONS. A list of hotels is
provided.
